Compare Israel's four kupot cholim — Clalit, Maccabi, Meuhedet, Leumit — across the dimensions that matter most to Olim.
Israel's universal healthcare system requires you to enroll in one of four kupot cholim (literally "sick funds" — they function as integrated HMOs). The basic coverage basket (sal briyut) is identical by law across all four, so the choice is not about what is covered — it is about access patterns, supplemental insurance quality, and fit with your specific needs. Most Olim default to whichever kupa their ulpan or absorption-center coordinator pointed them to. Some end up regretting it.
The five dimensions that actually differentiate kupot: (1) supplemental insurance tier quality (each kupa offers 2–3 tiers of bituach mashlim with different price points and coverage); (2) clinic density in your neighborhood (does Clalit have a clinic 5 minutes from your apartment, or is the nearest one 30 minutes by bus?); (3) hospital partnerships (which hospitals have priority admission, which require pre-approval); (4) specialist availability without referral (some kupot let you see specialists directly; others require GP referral); (5) English-speaking doctor density (especially important for mental health, pediatrics, complex specialties — varies dramatically by city).
For Olim specifically, the comparison tool addresses common concerns: mental health coverage and English-speaking psychotherapist availability (Maccabi and Meuhedet generally lead here), English-speaking pediatricians (especially in Jerusalem and Modi'in where Olim concentrate), dental coverage for children (basic dental is covered for under-18 universally; supplemental for adults varies), complementary/alternative medicine (acupuncture, chiropractic — all four offer some, with different inclusion criteria).
New Olim get a special 14-day window after Aliyah during which they can switch their initial kupa immediately at any post office — use it to actively choose rather than default. After that, regular residents can switch up to twice in any 12-month period; transfers take effect on one of six fixed dates per year (Jan 1, Mar 1, May 1, Jul 1, Sep 1, Nov 1) with a 45–105 day administrative lead. - All four health funds provide the same basic health basket mandated by the National Health Insurance Law. The differences are mainly in supplemental plans.
- New olim can switch their initial kupa within 14 days of Aliyah at any post office without waiting for a transfer cycle. After that, residents can switch up to twice per 12-month period; transfers take effect on one of six fixed dates per year (Jan 1, Mar 1, May 1, Jul 1, Sep 1, Nov 1) and take 45–105 days from application.

Frequently asked questions
You can switch kupot up to twice in any 12-month period. Switches take effect on one of six fixed dates per year — Jan 1, Mar 1, May 1, Jul 1, Sep 1, Nov 1 — depending on when you submitted the request (lead time is typically 45–105 days). Submit at any post office branch with your teudat zehut, or online through Bituach Leumi (Hebrew only). New Olim get a special 14-day carve-out: within two weeks of Aliyah you can change your initial kupa immediately at any post office, no waiting cycle. Existing prescriptions and basic medical records transfer to the new fund automatically.
Bituach mashlim is optional supplemental coverage on top of the basic basket. Typically covers: choice of doctor for surgery, alternative medicine, expanded dental, eye care, second opinions abroad, expanded mental health. Costs roughly ₪80–₪200 per adult per month depending on tier and kupa. For most Olim families, worth it — especially the choice-of-surgeon coverage. Top-tier supplemental insurance (the equivalent of "premium tier" in your home country) typically runs ₪300–₪500 per adult per month and adds private hospital room access and expanded coverage abroad.
Maccabi has historically been strongest for English-speaking specialists in Tel Aviv and Jerusalem. Meuhedet is strong in Jerusalem specifically. Clalit has the largest network overall but English coverage varies enormously by city — strong in Jerusalem and Tel Aviv, weak in periphery. Leumit is smallest and most variable. Best practice: research specific clinics near your address rather than choosing a kupa based on reputation alone.
For most Olim: no, the Israeli system is excellent and the kupa + bituach mashlim combination provides better coverage than most foreign private plans. Exceptions: if you travel internationally frequently and want consistent coverage abroad, or if you have a specific specialist relationship in your home country you want to maintain. Some Olim keep "evacuation insurance" (a subset of travel insurance) for medical evacuation back to home country in extreme cases — this is cheap (~$200/year) and reasonable.
The basic basket cannot deny coverage based on pre-existing conditions — by law you are entitled to full coverage from day 1 of residency. Bituach mashlim (supplemental insurance) typically has waiting periods for pre-existing conditions: 6–12 months before specific elective procedures are covered. Some conditions (cosmetic procedures, fertility treatments above a baseline) have longer waits. New Olim should enroll in bituach mashlim immediately on arrival to start the waiting-period clock.
